The Holy Cross Physician Partners (HCPP) network is comprised of more than 300 physicians throughout the community. The goals of HCPP are summarized in something known as "the triple aim," which was first articulated by Donald Berwick, MD, former acting CMS administrator. The three aims are (1) improving the experience of care, (2) improving the health of populations and (3) reducing per capita costs of healthcare.

Achieving these goals can only be accomplished by aligning health care purchasers and providers in a new and different way. Insurance companies must be willing to share information and change payment methods.
