Diabetes Self-Management
Holy Cross Health is proud to offer the Diabetes Self-Management Education & Support/Training (DSMES/T) Program to individuals who have diabetes.
Our DSMES/T program includes individual and group education. During the initial one-on-one session with a Certified Diabetes Care & Education Specialist, you will create a plan and set goals that fit your needs and lifestyle. In the group setting, you will learn ways to manage your diabetes while finding other people who have the same concerns and challenges as you.
Group Class Topics include:
- Healthy Eating
- Being Active
- Blood Sugar Monitoring
- Taking Medications
- Problem Solving
- Reducing Risks
- Healthy Coping
We also offer follow-up appointments to make sure you feel confident managing your diabetes and are achieving your goals.
Program Eligibility
You may be eligible for the program if you meet the following criteria:
- Diagnosis of diabetes
- Have one of the following blood test results:
- Hemoglobin A1C: ≥ 6.5%
- Fasting blood glucose: ≥ 126 mg/dL on 2 different occasions
- Oral glucose tolerance test: ≥ 200 mg/dL on 2 different occasions
- Random glucose test: > 200 mg/dL with symptoms
- Referral from your healthcare provider managing your diabetes
