If you forget your password, click on “Forgot Your Password”, and then enter your first name, last name and last 4 digits of your social security number. The system will prompt you to answer the security question you selected at the time of registration and allow you to choose to have the password e-mailed to you or displayed on screen.
The application is multi-layered and is therefore treated by some systems as a “pop-up”. If you have a “Pop-Up Blocker” installed on your computer, it will prevent the application from displaying. Please disable the Pop-Up Blocker and you should be able to access the application. You may reinstall the Pop-Up Blocker after the application has been completed and successfully sent.
If you receive an error message, it is because you are missing required information. The most common errors are not entering phone numbers or social security numbers in the proper format. All phone numbers need to be listed with a 3 digit area code followed by a hyphen (-), followed by 3 digits, followed by a hyphen (-) and then the remaining 4 digits, example: 954-771-8000. The same format is needed for the social security number, example: 012-34-5678.
You can save your information by clicking on “Save Now” in the small rectangular box located on the far right as you fill out the application. Once you have the missing information, you may log back in with your user name and password to complete the application.
If you are using an Apple device with the Safari browser and you are experiencing problems with the online application process, we recommend that you download either Internet Explorer for Mac or Firefox for Mac and attempt to complete the process using one of these browsers.
